Provider listing removal

Ava Health has discontinued its public provider directory. The directory previously indexed publicly available information from the National Plan and Provider Enumeration System (NPPES NPI Registry), a public database maintained by the Centers for Medicare & Medicaid Services (CMS). We never displayed email addresses, personal phone numbers, or any data outside of NPPES.

Individual provider listing pages have been removed and now return a 404. If you previously found your information listed here and have any remaining concern — including about search engines that may still show a cached or indexed copy — email us and we'll help.
