Directory data (discontinued).This Site previously published provider listings built from the National Plan and Provider Enumeration System (NPPES) NPI Registry, a public dataset published by the Centers for Medicare & Medicaid Services. That data included provider names, credentials, specialties, and practice locations that providers themselves submitted to the federal registry. We did not create this data and did not collect it from the providers directly. The directory has been discontinued and individual listing pages have been removed; see our opt-out page for details.

Information you give us. If you contact us, sign up for job alerts, or submit a form, we collect what you provide, such as your name, email address, and message.

Information collected automatically. Like most websites, we collect basic technical and usage data through cookies and analytics, such as your IP address, browser type, pages viewed, and referring URLs. This helps us understand how the Site is used and keep it working.

How we use information

We use information to operate and improve the Site, respond to your inquiries, send job alerts you request, maintain security, and meet legal obligations. We do not sell your personal information.

If you were previously a listed provider

Ava Health has discontinued its public provider directory. Individual listing pages, which reflected public NPPES registry data, have been removed and now return a 404. If you previously found your information listed here and have any remaining concern, see our opt-out page or email info@avahealth.co. Removal from our Site does not change the underlying federal NPPES record.

How we share information

We share information only with service providers that help us run the Site (such as hosting and analytics providers), and when required by law or to protect our rights. We do not sell personal information.
